Participation is voluntary. The administrator will use private responses to plan activities and review community interests. Responses are not displayed publicly. This consent does not cover a separate research study or publication of identifiable responses. Contact the administrator using the Contact page for questions.
All questions are optional. Answer at least one to submit. Do not include names, contact details or sensitive information in answers.
No member or subscriber identity is attached to this response. Free-text answers may still identify you. Because we cannot reliably identify your response later, individual withdrawal after submission is unavailable. Hosting logs may separately record connection details.
